What is a Stress Test?

A Stress Test helps to understand how well your heart is working during heavy activities. This test is usually performed when a person is on a treadmill or stationary cycle while an ECG device monitors your heart rate, breathing, blood pressure and heart rhythm.

What are the purposes of this test?

This test is a very common way to detect a heart issue. Your doctor might perform this test for different reasons, such as:

  • To check how properly your heart pumps blood.
  • To determine whether your heart is getting enough blood supply.
  • To determine how well your heart is doing in comparison to others of your gender and age while doing physical exercise.
  • To see if the symptoms include wheezing, shortness of breath only appears while doing physical activities.

This test also helps the doctors to easily understand if you are having heart issues, including:

  • Issues related to heart valves or muscles.
  • Inadequate blood supply to your heart muscles
  • Electrical stability of your heart while doing exercise and resting.

This test also helps the doctor to understand whether you need additional testing to confirm a diagnosis.

How does the exercise stress test work?

For a cardiac stress test, the heart needs to pump faster and harder. Most commonly, this test is done while walking on a treadmill, or pedaling on a stationary cycle. During this test, you doctor will analyze your:

  • Heart rate
  • Breathing rate
  • Oxygen levels
  • Electrical signals of your heart
  • Blood pressure
  • How well your heart is pumping in comparison to others of your age and sex

You need to get this test to detect certain heart problems, such as:

  • Coronary artery disease
  • Heart valve disease
  • Congenital heart disease
  • Congestive heart failure
  • Hypertrophic cardiomyopathy

This test is also required if you have a high risk occupation including professional athlete and pilot. People who are having symptoms of heart conditions may also require this test. The symptoms may include:

Generally, doctors suggest this test to:

  • People who are about to start exercising.

  • People who have diabetes or other health conditions that increase the chance of developing a heart disease.
  • People who are about to get a non-cardiac surgery, and to determine if there is any risk of complications.

What are the types of stress tests?

There are a few types of stress tests which depend on a person‘s requirements. The common types are:

1. Exercise stress test: During this test, your doctor will examine your heart rate, breathing, blood pressure, and how tired you are while doing different types of physical activities. Here are a few things which you can expect during an exercise stress test:

  • Some electrodes will be attached to your chest to analyze your heart rate.
  • First, you will be required to walk slowly on the treadmill.
  • After that the speed of the treadmill will gradually increase.
  • You might also be required to walk in an inclined position.
  • At the end of this procedure, you might be required to breathe into a mouthpiece to examine how much air you exhale.
  • After that, you will be required to lie down while your blood pressure will be checked.

The whole procedure generally takes 10 to 15 minutes. However, if you are feeling lightheadedness, fatigue, chest pain, irregular heart beat, low or high blood pressure while doing this test, then your doctor may stop the test. Your doctor may also stop the test if the ECG device detects any unusual readings.

2. Stress test without any exercise: This test is applicable for those who are not able to do exercise. The medical professional will use certain medications in order to generate the same effect.

During this test, the medical professional will attach the electrodes to a person’s chest and insert the medication through the arm with the help of an IV line. This procedure may take up to 15 to 20 minutes. The medication is used to deliver the same effect to the heart that takes place while exercising.

3. Nuclear stress test: This test is commonly known as nuclear heart test and radionuclide scan, which provides accurate and detailed information about your heart.

This test is similar to the exercise stress test, but in this test, the doctor will insert a tracer dye into the arm which will highlight the blood flow and heart on an image. This can also help to understand if there is any space of the heart where blood is not running, which may indicate a blockage.

How to prepare for the test?

Before performing the test, your doctor will perform a physical test and might ask you about your medical history. At this time, make sure to tell your doctor if you experience any symptoms that make it harder for you to exercise. Also, make sure to let them know if you have arthritis.

Your doctor might advise you to stop drinking and eating caffeinated products three hours before the test, and also avoid smoking. If you are currently on medications then ask your doctor for specific guidance. On the day of the test, make sure to wear loose and comfortable clothes and shoes.

What will be the interpretation of the test results?

If your test result is normal, then the interpretation depends on the cause why you went for the test.

  • In case of heart valves evaluation, a normal test result may indicate that the symptoms are not related to a heart condition and might require additional testing to determine the exact cause.
  • If your doctor recommended this test to determine the risks before a surgery then a normal test result indicates that your heart is healthy and you can go for the surgery.
  • In the case of monitoring a heart condition, a normal test result indicates that your body is responding well to the treatment you are receiving.

If your test result is abnormal, then it might indicate that you have a heart problem.

  • If any abnormalities appear during the initial stages of the test then additional testing is required, such as nuclear stress test, cardiac catheterization, stress echocardiogram, and CT coronary angiography.
  • Abnormal test results may also indicate that your heart is not capable of exercise and surgery.

If you have signs of mild heart disease then your doctor would ask you to improve your lifestyle, including weight management and quitting tobacco. They might also recommend certain medications to manage high cholesterol levels, blood pressure and blood sugar levels to reduce the risk of complications.
